Yizkor Form

Dear Friends,

This year, Chabad of Prescott will print a beautiful Yizkor Memorial Book honoring the cherished memories of our departed loved ones.

Yizkor will be recited on Yom Kippur at 12:00 noon. The memorial book will also be used at our Yizkor services throughout the Jewish year, and submitted names will be read on Yom Kippur, Shemini Atzeret, Passover, and Shavuot.

You may honor a loved one with:

• A full-page dedication â€“ $180
• A half-page dedication — $100
• An individual memorial name — $18

Giving tzedakah in memory of a loved one is a beautiful way to bring continued merit to their soul and ensure that their legacy continues to inspire goodness.
